Please follow these instruction to submit a copy of your 2021 tax return for your business. Complete this by Monday, January 27, at 8:00 a.m.
Follow these steps to upload your document:
1. Go to the application portal.
2. Click "authorize".
3. Click the "Open" button on the first box reading "Additional Application Changes".
4. Click "Select a file" to find the document on your device. After you upload click "Mark as Complete".
5. If your document uploaded successfully, you will be taken this screen (below), and the request box should read "Complete." On this screen, click on the button that says "Submit."
6. After you hit submit, you should see this screen and a message that says, "Form has been submitted."
If you do not complete these steps by the deadline your application will not be processed in this grant round. You may have the opportunity reapply in future funding rounds.
